Flexi Layouts 3 works on PC or MAC on the following Dreamweaver versions: CS4, CS5, CS5.5, CS6, CC, CC 2014, CC 2014.1, CC 2014.1.1, CC 2015. The page where you use the form must have an HTML5 Doctype because it has datatype attributes that are HTML5 specific.
